宫颈疾病论文:宫颈锥切术770例的临床分析
【中文摘要】:探讨宫颈锥切术用于宫颈疾病治疗的适应证、宫颈锥切术的优缺点、用于各种宫颈疾病的治疗中是否存在过度治疗的问题。方法:回顾性分析2009年1月~2010年12月吉林大学白求恩第二医院门诊及病房宫颈锥切术770例,对宫颈锥切术前进行各项检查、术前诊断及术后病理结果进行回顾性分析。总结诊断性和治疗性锥切术的例数,以及宫颈环形电切术(loop electrosurgical excisional procedure)和冷刀锥切术(cold knife conization)所占的比例,归纳行LEEP和CKC的理由,分析评价是否符合宫颈锥切术(LEEP和CKC)的适应证。按照LEEP和CKC的治疗规范,评价符合及不符合治疗性宫颈锥切术指征的例数及所占的比例。总结不符合治疗性宫颈锥切术的病例数,评价是否存在过度治疗。分析过度治疗的原因。结果:共770例,诊断性锥切术18例,治疗性锥切术752例。LEEP术700例(其中诊断性18例),冷刀锥切70例(均为治疗性锥切术)。术前有宫颈活组织检查的病例408例中,术后病理升级33例(%),其中术前为宫颈良性病变术后病理结...
