院感染管理控制组织及实施方案
　　预防和控制医院感染是每一位医务工作者应履行的社会责任；是医院生存开展以及医务人员的自身平安的需要。因此，预防和控制医院感染是医院管理体系中不可缺少的重要局部，它不只是某个人或某个部门的工作职责，而是涉及全体医务人员的系统工程。医院感染管理是医疗质量管理的重要组成局部。今年医院感染管理工作重点仍以医院感染病例发现、登记报告；重点科室的环境卫生学监测；医疗废物的管理；消毒药械、一次性医疗用品管理；法定传染病的管理；医务人员手卫生的管理；职业暴露处理管理，以及感染科、手术室、产房、新生儿抢救室等重点部门及高危科室的管理，协助质控、药剂科对抗菌药物临床应用进行管理。

　　医院感染监测主要包括感染病例监测、环境卫生学监测、消毒灭菌效果监测。
　　1．医院感染病例监测：监测有全面综合性监测和目标性监测两种方法。目前我院实行全面综合性监测，主要由临床各科室负责人负责感染病例的发现、登记和报告，由副组长收集感染卡片进行统计分析。方案用2年时间完成院感根底发病率调查，为目标性监测做准备。如果病区发生医院感染流行趋势时，应立即报告医院感染管理科，医院感染管理科根据情况进行流行病学调查和环境、物品监测，确定是否为流行，找出流行的原因，并提出整改措施。细菌室负有向医院感染管理科汇报病原体别离和耐药情况的责任，遇到医院感染爆发流行，应承当相关的监测工作。
　　2．环境卫生学监测： 空气、物体外表和医务人员手的监测：主要由医院检验科完成采样工作，一般1-2月监测一次，纳入质量控制考核标准。
　　3． 毒剂、消毒灭菌效果的监测： 〔1〕使用中消毒剂每季度进行一次微生物监测，灭菌剂每月监测一次，由医院感染管理副组长负责；使用中的浓度监测由科室完成，并做好记录。 〔2〕消毒灭菌效果的监测：主要是高压锅的监测，每月由供给室进行一次微生物监测，医院感染管理小组定期抽查。

　　1．消毒产品的管理：消毒产品包括消毒剂、消毒器械、卫生用品和一次性使用医疗用品，医院感染管理科参与消毒产品的购入、使用和用后处理的监督指导，具体包括产品购入的质量控制，使用中的监测和一次性医疗用品用后处理的检查、监督和指导。
　　2．抗菌药物合理应用的管理：协助药剂科对临床抗菌药物合理应用指导进行督导，定期检查临床科室抗菌药物应用的情况，反响检查结果，并根据病原体对抗菌药物的耐药情况规划全院抗菌药物应用，制定限用和轮换制度；临床科室应根据国家有关抗菌药物合理应用的指导原那么和标准要求，结合本科室的具体情况制定本科室的抗菌药物合理应用细那么，接受医院抗菌药物合理应用指导小组的监督、检查和指导。
　　3．医院医疗垃圾的管理：根据卫生部和国家环保总局发布的?医疗废物处理条例?、?医疗机构医疗废物管理方法?、?医疗废物管理行政处分方法?、?医疗废物分类目录?、?医疗废物专用包装物、容器标准和警示标识规定?等制定医院医疗废弃物的管理规定，临床医技科室应遵照医院有关医疗废物处理的规定在垃圾产生的开始进行分类收集、运送，医院感染管理科定期检查和监督垃圾的分类收集运送情况。

　　四．消毒、灭菌与隔离的原那么
1、医务人员必须遵守消毒灭菌原那么，进入人体组织或无菌器官的医疗用品必须灭菌；接触皮肤粘膜的器具和用品必须消毒。 用过的医疗器材和物品，应先去污染，彻底清洗干净，再消毒或灭菌。所有医疗器械在检修前应先经消毒或灭菌处理。
　　2、根据物品性能选用物理功化学方法进行消毒灭菌。耐湿物品灭菌首选灭菌法；手术器具及物品、各种穿刺针、注射器等到首选压力蒸汽灭菌；油、粉、膏等首选干热灭菌。
