摘 要
目的:探讨治疗环状痔的新术式。目前,微创化、局限化是近代外科手术的发展趋势。为了在保证疗效的前提下,既力求一次性消除病症,又最大限度地保护肛管上皮及ATZ(直肠肛管移行区)的完整性和连续性,维持肛门的正常功能,在导师指导下, 根据肛门解剖及生理特征,结合近年来国内外治疗环状痔的新进展,设计了保留肛管上皮及ATZ治疗环状痔的术式。方法;选择环状混合痔患者50例,随机分为治疗组25例,分段齿形结扎对照组25例。治疗组采用肛管上皮及ATZ保留术,对术后刨面愈合时问,便秘、漏液漏气等并发症,及肛管口径大小变化、最大肛管静息压变化等指标进行了观察统计。结果:运用肛管上皮及ATZ 保留术治疗环状痔,术后创面愈合时间缩短,便秘及肛门渗液等并发症减少,肛管静息压较术前明显降低,肛管口径大小基本无变化。结论:肛管上皮及ATZ保留术治疗环状混合痔可缩短创面愈合时间,保持肛门正常的生理结构及功能,减少术后并发症。

ABSTRACT
Objective:To evaluate a New operation produre for the treatment
of circular mixed hemorrhoids;not only obliterating symptom disposably,but also protecting the integrality and furthest of anal canal
Methodds:Fifty adult patients witll circular mixed hemorrhoids who need operation were divided randomly into two groups(n225).The therapy group adopting the the operation of preserving anal canal epithelium and ATZ,the control group employing the operation of segmental dentate ligation;observing and statisticing the indexes of heallhing time of the operative plications,and the variation
of caliber of anal canal and static pressure of anal canal.
