　 【摘要】 目的：探讨多排螺旋CT在儿童肺挫裂伤诊断中的应用价值，旨在为临床诊断肺挫裂伤患儿提供依据。方法：回顾性分析于2016年8月-2018年1月在筆者所在医院接受治疗的200例肺挫裂伤患儿影像学资料，所有患儿均接受多排螺旋CT和传统X线片检查。对比两种诊断方法诊断符合率，分析两种方法影像学表现和合并症情况。结果：%，%，差异有统计学意义（P<）。多排螺旋CT检查显示，合并肋骨骨折患者56例，胸骨骨折5例，胸腔积液62例，气胸38例，左肾挫裂伤12例，心包积液3例。结论：多排螺旋CT可为肺挫裂伤患儿的治疗提供更多诊断信息，提高肺挫裂伤诊断符合率，明确肺挫裂伤患儿合并症，是儿童肺挫裂伤重要诊断方法。

　　 [Abstract] Objective： To explore the value of multi-slice spiral CT in the diagnosis of pulmonary contusion and laceration in children， in order to provide a basis for clinical diagnosis of pulmonary contusion and laceration in children. Method： The imaging data of 200 children with pulmonary contusion and laceration treated in our hospital from August 2016 to January 2018 were retrospectively analyzed. All patients underwent multi-slice spiral CT and traditional X-ray examination. The coincidence rate of the two diagnostic methods was compared， and the imaging manifestations and complications of the two methods were analyzed. Result： The coincidence rate of CT examination was %， which was significantly higher than % of X-ray examination， and the difference was statistically significant （P<）. Multi-slice spiral CT examination showed that 56 cases with rib fracture， 5 cases with sternal fracture， 62 cases with pleural effusion， 38 cases with pneumothorax， 12 cases with left kidney contusion and laceration， and 3 cases with pericardial effusion. Conclusion： Multi-slice spiral CT can provide more diagnostic information for the treatment of children with pulmonary contusion and laceration. It can improve the diagnostic coincidence rate of the pulmonary contusion， and identify the complications of pulmonary contusion in children. It is an
