1、全市各级健康教育机构和网络健全，人员、经费落实，机构能够承担起业务技术指导中心的职责，社区健康教育网络能够发挥作用。（1分）
证实材料：
（1）市与办事处、办事处与各社区、办事处与驻街单位的健康教育网络齐全。
（2）落实经费必须有帐目和凭证证明（发票）。
（3）各级健康教育机构的技术指导作用，用记录本来证明。
2、各街道、社区（居委会）、单位有专（兼）职健康教育人员，形成健康组织网络，定期开展健康活动。（）（查活动记录等资料，）
证明材料：
（1）建立街道、社区、各驻街单位健康教育网络。
（2）定期开展健康教育活动记录本。（专人填写）
3、辖区各单位使用市统编的“健康教育工作记录本”，并根据单位情况逐项填写完整。市、街道及各系统、单位均应随时提供近三年来的完整资料。（）（查工作记录本及有关资料，）
