China) Abstract : Objective To explore the relationship between HBV infection and hepatic melastasis of colorectal cancer. Methods Three hundred and eighty-six cases with colorectal cancer were retrospectively analyzed from January 1996 to Decemer 2005 , and the prevalence of hepatic metastasis of colorectal cancer in patients with and without hepatitis B virus infection pared. Results In the HBV carriers, the prevalences of hepatic metastasis were % and that in non-HBV carriers was %, showing significant difference between the two groups (P<). Conclusion The prevalence of homochronous hepatic metastasis and heterochronous hepatic metastasis of colorectal cancer in patients with HBV infection is lower than that of non-HBV infection metastasis of colorectal cancer is infrequent in pateints with HPV infection. Key words : Hepatitis B virus; Colorectal cancer; Hepatic metastasis 有报道临床确诊为结直肠癌时,已有 20% ~ 40% 发性肝转移[ 1]。为探讨乙肝病毒感染与大肠癌肝转移中所起作用,本文对我院自 1996 年1月~ 200 5 年 12 月收治的大肠癌患者 386 例,有 82 例并发肝转移,其中大肠癌并乙肝病毒感染者发生肝转移 5例,现对其回顾性分析、探讨。 1 资料与方法 一般资料 386 例患者中,男 234 例,女 152 例,年龄 22 ~ 86 岁,平均 61 岁。 HBsAg 阳性者(即感染组) 58 例,阴性者(即非感染组) 328 例。 治疗 82 例肝转移者 58 例行原发灶切除。肝转移灶同时切除 20 例,微波固化 9例,无水酒精注射 21 例,单独或同时行肝动脉插管化疗 32 例。 统计学方法采用 统计软件包χ2检验进行。 2结果有 82 例并发肝转移,转移癌数目不等,其中 HBV 感染者并发肝转移占 %(5/58) ,非感染者并发肝转移占 %(77/328) 。术前 B超或/并 CT 检查发现肝转移 68 例,其余 14 例术中探查时病理确诊; 82 例大肠癌肝转移者 AFP 均正常,总的肝转移率 % ( 82/386 )。肝转移部位:左右两叶 18 例,左叶 28 例,右叶 36 例;单个转移结节 16 例, 2~4个转移结节 34 例, ≥5个转移结节 32 例;其它部位(包括肺、骨等的远外转移及腹膜种植转移)转移情况,感染组 19%
