US与MRI检查在妊娠中晚期胎儿或胎盘异常诊断中的应用比较

摘要：目的：探讨超声（ultrasound,US）与磁共振成像（magnetic resonance imaging ，MRIMRI？
)在妊娠中晚期胎儿或胎盘异常诊断中的应用价值。方法：回顾性分析了2017年1月至2019年6月海口市人民医院和海口市妇幼保健院经超声检查怀疑存在胎儿或胎盘异常的140例孕妇的盆腹部US及MRI影像资料并比较其在胎儿或胎盘异常中的诊断价值。结果：在140例US检查怀疑胎儿或胎盘异常的孕妇中，MRI检查结果有30例为正常，超声检查结果与MRI检查和病理或出生随访结果比较差异存在统计学意义（P＜），而MRI检查结果与病理或出生随访结果更为接近，差异无统计学意义（P＞）；在超声检查怀疑存在胎儿或胎盘异常孕妇中，MRI检查发现胎盘植入的检出率更接近病理或出生随访结果（P＞），超声检查与MRI检查结果间存在统计学差异（P＜）；US和MRI检查与病理或出生随访结果相比在脑发育不良、脑室发育异常、胼胝体发育不良、胼胝体缺如、脑血管畸形、Dandy-Walker、枕大池增宽、眼球发育异常、视神经发育异常、蛛网膜囊肿、唇腭裂、颈部淋巴管囊肿、肺内错构瘤、泌尿系积水、双肾体积缩小、脊柱发育不良、手缺如、脐带绕颈、前置胎盘和瘢痕子宫的检出率差异均无统计学意义（P＞）。结论：MRI检查能够更清晰的显示妊娠中晚期胎儿或胎盘异常，MRI检查可以降低US在妊娠中晚期胎儿或胎盘异常诊断中的假阳性率，US联合MRI检查可有效提高妊娠中晚期胎儿或胎盘异常的诊断准确率。
