    （四）有内镜消毒灭菌设施，医院感染管理符合要求。
    （五）拟开展风险高、过程复杂、难度大，按照四级手术管理的泌尿外科内镜诊疗技术（附件1）的医疗机构，在满足以上基本条件的情况下，还应满足以下要求：
1、三级医院，开展泌尿外科诊疗工作不少于5年，近5年累计完成泌尿外科内镜手术不少于2000例，其中累计完成按照四级手术管理的泌尿外科内镜手术不少于500例或累计完成按照三级手术管理的泌尿外科内镜手术（附件2）不少于800例。技术水平在本地区处于领先地位。
2、具备满足危重患者救治要求的重症监护室。
3、具备满足实施按照四级手术管理的泌尿外科内镜手术需求的临床辅助科室、设备和技术能力。
二、人员基本要求

（一）医师。
1、拟开展泌尿外科内镜手术的医师，应当同时具备以下条件：
    （1）取得《医师执业证书》，执业范围与开展泌尿外科内镜技术相适应的临床专业。
    （2）具有5年以上泌尿外科疾病诊疗工作经验，具备主治医师以上专业技术职务任职资格。目前从事泌尿外科诊疗工作，累计参与完成泌尿外科内镜手术不少于200例。
    （3）经过泌尿外科内镜诊疗技术系统培训并考核合格。
2、拟独立开展按照四级手术管理的泌尿外科内镜手术的医师，在满足上述条件的基础上，还应满足以下条件：
    （1）开展泌尿外科诊疗工作不少于10年，具有副主任医师以上专业技术职务任职资格。累计完成泌尿外科内镜手术不少于500例；其中按照三级手术管理的泌尿外科内镜手术不少于250例。
