聊城市开办药品零售企业审批方法

第十条 药品零售企业的设置实行一证一址制度，以合理布局、方便群众购药为原那么，在审批条件上适当考虑城乡差异因素。
第二章 申办条件
第十一条 药品零售企业申办的条件：
〔一〕具有与所经营药品相适应的营业场所、设施设备以及卫生环境。营业场所和仓库应配备检测和调节温湿度的设备，经营冷藏药品的，应有冷藏设备，并建立温湿度记录。
〔二〕零售企业的营业场所及仓库面积要求：
1、经营非处方药品的零售企业，用于药品经营的营业场所使用面积应符合以下要求：
⑴县〔含〕以上城区不少于40平方米。
⑵县以下农村地区不少于20平方米。
⑶在车站、宾馆及其他商场、超市等特定区域的，必须设置独立的区域，营业区域使用面积不少于20平方米。

2、经营处方药品的零售企业，用于药品经营的营业场所使用面积符合以下要求：
⑴县〔含〕以上城区不少于100平方米。
⑵县〔含〕以上城区零售连锁门店不少于60平方米。
⑶县以下农村地区不少于40平方米。
3、企业可根据需要设置与经营规模相适应的药品仓库，其地址应与营业场所地址一致或相连。未设置仓库的，药品不得放置在货架、柜台之外的场所。
〔三〕零售企业的人员要求：
1、零售企业、企业法定代表人、企业负责人、企业质量负责人、企业质量管理机构负责人或质量管理人员无?药品管理法?第76条、第83条规定的情形。
2、零售企业负责人应熟悉国家有关药品管理的法律、法规、规章和所经营药品的知识，并具备以下条件：
⑴非处方药店企业负责人应具有高中〔含〕以上学历；
⑵处方药店企业负责人应具有药学或药学相关专业中专〔含〕以上学历。
3、零售企业质量负责人应具备以下条件：
⑴经营非处方药的县〔含〕以上城区的零售企业质量负责人应具有药师以上技术职称；连锁门店和县以下农村药品零售企业质量负责人应具有药士以上技术职称，或具有中专以上药学或相关专业学历。
