2、补卡：携带本人有效证件（身份证、户口簿等），至邻近的区县医保中心申请办理，区县医保中心可当场予以办结。 也可委托他人代为办理，被委托人在办理时需携带本人及参保人的有效证件（身份证、户口簿等）。
注：参保人有正常使用的《社保卡》的，不再补换《医保卡》。
二、《门急诊就医记录册》（如下图）
（一）申请条件：
参保人【不含非城镇（即农村）户籍外来从业人员】应携带《医保卡》以及本人有效证件至邻近的市、区县医保中心申领。
（二）补办流程：
《门急诊就医记录册》遗失、损坏、涂改、缺页或用完的，参保人至邻近的市、区县医保中心办理新册，办理时应携带本人《社保卡》或《医保卡》、《门急诊就医记录册》（遗失除外）以及本人有效证件。参保人可以委托他人代为办理，被委托人在办理时另需携带本人及参保人的有效证件（身份证、户口簿等）。
三、上海医保政府网站个人密码申请
参保人可在办理《医保卡》《门急诊就医记录册》时，可一并申请个人密码，次日生效。参保人可登陆上海医保网“个人密码页面”（?lm=0），输入证件号和个人密码。
