磁共振胰胆管成像在胰胆管疾病中的诊断价值

[ 摘要] 目的探讨磁共振胰胆管成像( MRCP) 的临床应用价值。方法采用重 T2 加权多次激发对 100 例临床拟诊胆道梗
阻的患者进行 MRCP 检查, 并经手术病理或其他影像学或临床资料证实。结果恶性梗阻 28 例, 其中胰头癌 9 例, 壶腹癌 6
例, 胆管癌 10 例, 原发性肝细胞癌直接侵犯 3 例; 良性梗阻 72 例, 其中胆总管结石 33 例、胆囊切除术后狭窄 5 例、胆囊炎或结
石 26 例、慢性胰腺炎 4 例, 先天性胆总管囊肿 2 例, 硬化性胆管炎 2 例。本组定位诊断准确率为 89% , 定性诊断准确率 86% 。
结论 MRCP 对于胆道梗阻性病变的定位较为准确, 结合 MRI 平扫及资源像, 定性诊断也有一定的特异性, 是一种安全、无创
的检查手段, 能为临床诊治提供可靠依据。

[ Abstr act] Objective To evaluate the clinical application of MRCP in diagnosis of pancreaticobiliary duct dis2
