摘要

目的: 探讨天冬氨酸特异性半胱氨酸蛋白酶-1(Caspase-1)抑制剂对大鼠脑损伤后脑水
肿、细胞凋亡、IL-18表达的影响。

方法: 雄性SD大鼠170只,用改良的Feeney自由落体制备损伤模型, 随机分为正常组、
损伤组、治疗组。治疗组于损伤前后各30min 右侧脑室注射Caspase-1 抑制剂
(z-YVAD-fmk)。损伤组与治疗组分别于脑外伤后6、24、72、168h时间点断头取头。
用干湿重法检测脑含水量,免疫组化测定Caspase-1、 IL-18表达及TUNEL测定细胞凋
亡。

结果:损伤组脑损伤周围皮质组织 6h 出现水肿,随损伤时间的延长, 脑组织中水含
量于损伤后 3 天达高峰(±),后逐渐下降,至损伤后 7 天是仍高于正常组
(P<)。YVAD 组伤后各时间段脑组织含水量明显低于损伤组(P<)。且损伤
组脑损伤周围皮质组织 6h 即可见 Caspase-1、IL-18 少量表达,24h 达高峰分别为
(±、 ±),72h 表达已减少,至伤后 168h 已基本无 Caspase-1、IL-18
表达。YVAD 组各时间段脑组织中 caspase-1、IL-18 表达明显低于损伤组(p<0. 01)。
同时损伤组损伤周围皮层组织可见多量凋亡阳性染色细胞,YVAD 组凋亡阳性染色细
胞明显减少。

Abstract
Objective:
To investigate the effect of caspase-1 inhibitor z-YVAD-fmk on brain edema, neurocyte
apoptosis and IL-18 expression following traumatic brain injury in rats.

Methods:
One hundred and seventy male SD rats were randomly divided into normal group, injury
group and therapy group. Rat traumatic brain injury model was prepared by modified
Feeney free dropping objective. Therapy group injected with z-YVAD-fmk into the right
lateral ventricle 30 minutes before and after injury. All rats were decapitated at the
