    （5）申请人不能亲自办理的，可以委托他人代办。委托他人办理时，除提供上述规定的材料外，还需提供《生育证代办委托书》和被委托人身份证原件。
2、有以下情况之一的，还应提交相关资料原件及复印件：
    （1）一孩病残，需提交市州级卫生计生部门组织鉴定的鉴定结果。
    （2）无子女、收养一个孩子后要求生育，需提交夫妻依法收养子女的证明。
    （3）夫妻双方系归国华侨，需提交夫妻双方常住户口所在地县级以上人民政府负责侨务工作的机构出具的归侨证明。
    （4）夫妻一方是独生子女，独生子女一方父母亲户籍地不在申报乡（镇）、街道的，应提交父母亲户籍地村（居）出具的婚育证明，户籍地属外省的，需提供乡（镇）人民政府或者街道办事处卫生计生行政管理部门出具的婚育情况证明。
    （5）再婚夫妇，一方只有一个子女，另一方无子女。再婚夫妇，一方已生育两个子女，另一方未生育过子女，需提交再婚一方的离婚证、离婚判决书（调解书）或具有法律效力的协议书原件。有多次婚姻史的，要提交所有婚姻史变化的离婚判决书（调解书）或具有法律效力的协议书原件。再婚前属丧偶的，丧偶的一方需提交公安部门出具的户籍注销（死亡）证明，或医疗保健机构出具的死亡证明。
