    〔1〕属非医学需要选择性别人工终止妊娠的；
    〔2〕成心致婴儿死亡的；
    〔3〕自报婴儿死亡，但没有死亡证据、证明的；

〔三〕申请途径
申请人可采取多种渠道提交***申请。
1、申请人可通过所在村〔居〕民委员会提交申请，由村〔居〕计生专干将***相关材料提交所属乡〔镇〕人民政府或者街道办事处卫生计生行政管理部门受理。
2、申请人可向夫妻一方户籍地或者现居住地〔居住半年以上〕乡〔镇〕人民政府或者街道办事处卫生计生行政管理部门提交申请。
〔四〕所需资料
夫妻双方要求生育第二孩或再生育子女的，办理?生育证?时应提交以下材料：
1、根本资料：
   〔1〕夫妻双方的户口簿、结婚证、身份证原件及其复印件1 份；
   〔2〕夫妻近期2寸免冠合影照片1 张；
   〔3〕?生育证申请表?1 份。
   〔4〕夫妻双方户籍地所在村〔居〕民委员会出具的?婚育情况证明?。一方户籍地在外省的，需提供户籍地乡〔镇〕人民政府或者街道办事处卫生计生行政管理部门出具的婚育情况证明；

2、有以下情况之一的，还应提交相关资料原件及复印件：
    〔1〕一孩病残，需提交市州级卫生计生部门组织鉴定的鉴定结果。
    〔2〕无子女、收养一个孩子后要求生育，需提交夫妻依法收养子女的证明。
    〔3〕夫妻双方系归国华侨，需提交夫妻双方常住户口所在地县级以上人民政府负责侨务工作的机构出具的归侨证明。
