Radio receivers (TV/AM/FM) must be permanently marked with:
“This plies with Part 15 of the FCC Rules. Operation is subject to the condition that this device does not cause harmful interference.”
All other Digital devices (over 9 kHz) must be permanently marked in a conspicuous location with:
“This plies with Part 15 of the FCC Rules. Operation is subject to the following two conditions: (1) This device may not cause harmful interference, and (2) this device must accept any interference received, including interference that may cause undesired operation.”
FCC Declaration of Conformity Markings (applies to unintentional radiators that may connect to PC’s, receivers TV & Cable devices)

Digital devices (over 9 kHz) subject to FCC authorization under Declaration of Conformity (such as TV puter connecting devices) must be permanently marked in a conspicuous location with the FCC logo, Tradename, and Model Number.
