摘要
目的利用合理用药监测系统(presciption automatic screening system,)对高血压病治疗中的用药状况进行监测,分析不合理联合用药实例,并对不同药物联合用药进行分析,以提高高血压病治疗的合理用药水平。
方法登陆PASS临床药学工作站,对高血压病住院病人的用药医嘱连续监测两个月(2008年3月1日~4月30日)。获取联合用药的有问题医嘱(不合理联合用药)资料,经归纳、整理,按联合用药存在问题的性质(禁忌联用、不推荐联用、谨慎联用和关注联用)分类叙述。分析现在合理的联合用药。
结果共监测用药医嘱6784条,监测出问题医嘱2317条,其中存在配伍禁忌1条;存在不同程度药物相互作用115条:其中不推荐联用28条,谨慎联用72条,关注联用15条。在目前常用的4类药物(利尿剂、β受体阻断药、二氢吡啶类钙拮抗药和ACEI药)中,并对四类的两两联合进行分析。
结论 PASS系统数据库中将药物相互作用的严重程度分为4个级别并配以相应的颜色警示:禁忌联用(黑色)、不推荐联用(红色)、谨慎联用和关注联用(黄色),按此分级进行讨论。禁忌联用:表示两种药物在任何情况下均应禁用,不得给予同一个病人使用,警示灯为黑色。严重(不推荐联用):表示已有明确的有害药物相互作用,有潜在危险,警示灯为红色。中等(谨慎联用):需评估每个病人发生相互作用的危险,在必要时采取措施,警示灯为黄色。未定(关注联用):严重性尚未确定,应评估对病人的危害程度,必要时采取措施,警示灯也为黄色。

Abstract
