【摘要】目的探讨多层螺旋CT扫描及三维重建技术在寰枢椎损伤中的价值。方法回顾性分析39例经临床确诊的寰枢椎损伤患者的MSCT及DR资料。结果 39例患者中,骨折共50处,合并脱位9例,单纯寰枢关节半脱位10例。50处骨折及19例脱位DR确诊率分别为68%(34/50)、%(9、19);轴位图像确诊率分别为90%(45/50)、%(13/19);SSD图像确诊率分别为96%(48/50)、100%(19/19);MPR图像确诊率均为100%(50/50)、100%(19/19)。结论在寰枢椎损伤诊断中,普通X线检查漏诊率较高,MSCT是诊断寰枢椎损伤的首选方法,结合多种重建方法,有助于寰枢椎损伤的诊断。

[Abstract] Objective To evaluate the role of multi-slice spiral CT(MSCT ) and three-dimensional reconstruction in detecting atlantoaxial injury. Methods The MSCT and digital radiography (DR) data of 39 cases with atlantoaxial injury verified by clinical workups were retrospectively analyzed. Results Total 50 pieces of bone fractures was found in all of 39 cases, in which bination of dislocation was found in 9 cases and a simple atlas-axis joint half-dislocation were seen in 10 cases. DR could accurately determine the final diagnostic rates as high as 68%(34/50) in 50 pieces of bone fracture and %(9/19)in 19 cases of dislocation , and the rates of CT scan were 90%(45/50) and %(13/19),respectively;and surface shaded display(SSD) were 96%(48/50)and 100%(19/19),respectively;and multiplanar reconstruction(MPR) were 100%(50/50)and 100%(19/19),respectively.
