慢性乙型肝炎肝组织中PD-LI的表达与临床相关性的研究
摘 要
,并比较其与肝脏炎症分
级、纤维化分级、肝功能、酶学指标及病毒载量的相关性,以阐明其在乙型肝炎慢性化发生发展中的作用及临床价值,为以此类分子为靶标的免疫疗法提供参考。
研究方法
用免疫组织化学方法结合图像定量分析系统检测65例慢性HBV感染者(包括慢性乙肝病毒携带者5例,慢性乙型肝炎轻度20例,中度24例,重度16例)和5例正常对照组(肝血管瘤患者),并与肝组织炎症分级、纤维化分级、血清中丙氨酸转氨酶(ALT)、天冬氨酸转氨酶(AST)、总胆红素(TBIL) 及乙肝病毒载量(HBVDNA)进行相关分析。
结果免疫组织化学分析结果显示,正常对照组肝组织不表达或弱表达PD—LI,慢乙肝
肝组织中PD—L1呈阳性表达(尸<),且其表达量与肝脏炎症分级、纤维分级、TBIL、 ALT、AST呈正相关(r=,,,,,P<),与HBVDNA载量呈负相关(,P<)。
结论 ,有望成为慢

Method
The expression of PD-L 1 WaS detected by immunohistochemistry puter image quantitative analysis in the hepatic tissues of 65 patients、^ritIl chronic hepatitis B (including 4 groups of ASC,CHL,CHM,CHH,in which enrolled 5,20,24,1 6)and hepatic haemangioma patients of 5嬲normal the correlation analysis have been made between the inflammatory and fibrosis grading in the hepatic tissues and ALT,
AST,TBIL,HBVDNA load in serum. Results
The expression of PD-L 1 in hepatic tissues due to hepatitis B virus infection WaS significiently stronger than that in controls(P<).In all the subjects,the fluctuant level of PD—L 1 WaS positively correlated with inflammation and fibrosis grading of hepatitis
tissues,TBIL,ALT and AST level in serum(r=,,,,,P<),
and negatively correlated with serum HBVDNA load(r= 2,P<).
Conclusion
1nle increaSed PD-L I expression may probably involve in the genesis and progress of hepatitis B vires infection,which might hope to be new targets of immunity therapy of chronic hepatitis B.
