富血小板血浆联合自体脂肪移植多层次多隧道注射治疗哺乳后乳腺萎缩的临床观察

Conclusion PRP combined with autologous fat transplantation with multi-level and multi-tunnel injection has a good clinical effect in treating breast atrophy after breast-feeding, with fewer complications. The breast shape after operation is straight and straight, and the feeling is natural. It is worthy of clinical application. 现代女性以胸部丰满为美,饱满挺拔的乳房不仅能满足生理上的哺乳需求,也是女性魅力的良好体现。哺乳后乳腺萎缩是小乳症常见原因之一,临床上较为常见,其病因主要是哺乳前后女性体内激素的变化及哺乳消耗[1],导致乳腺萎缩变小、脂肪量减少,乳房整体形态欠饱满,有的也伴有皮肤松垂,严重影响了乳房的形态美观,越来越多的女性就医者希望通过手术方式改善哺乳后乳腺萎缩造成的乳房形态欠佳。隆乳手术临床上常用的方式有假体隆乳及自体脂肪隆乳[2]。假体隆乳在临床上应用时间较长,可以达到良好的隆乳效果,但也存在包膜挛缩、假体移位等风险,且有部分就医者排斥使用假体[3]。自体脂肪隆乳手术操作简便,来源稳定可靠,隆乳效果肯定,受到了较多手术医师及就医者的青睐,但也存在吸收程度不定及形成结节、囊肿等风险[4]。富血小板血浆(Platelet-rich plasma,PRP)是一种新型自体组织材料,临床应用及实验研究表明PRP可以促进移植脂肪存活,降低吸收率[5]。有鉴于此,2016年3月以来,采用PRP结合自体脂肪移植多层次、多隧道注射治疗哺乳后乳腺萎缩,取得满意的术后效果,现报道如下。
1 临床资料
本组共72例(144侧乳房)要求改善哺乳后乳腺萎缩的女性就医者,年龄23~41岁,,均为停止哺乳后出现乳腺萎缩、变小,乳房欠饱满,自觉乳房形态不佳。所有就医者均采用PRP结合自体脂肪移植多层次、多隧道注射隆乳。所有就医者均无假体隆乳或注射隆乳史,脂肪供区充足,排除乳房疾病者,均无手术禁忌证。
